Milford health provider receives $675K grant for medical outreach

MILFORD, DE- Those suffering from chronic conditions in Sussex County will now be better informed on how to manage their conditions.
Nearly 675 thousand dollars is heading to the Education Health Research International at the Milford Wellness Village to assist older homebound adults in knowing their conditions with self-management education. The money was granted to the Education Health Research International which seeks to increase the available info for home-treatment patients, on when they need to go to an in-person clinic for help. It will help to support the 1,000 seniors the center serves, and teach them what to look out for when dealing with chronic illness.
“People have complications and are unable to get the care they need so learning to self-manage and learning when they need to get connected with healthcare providers is hugely important,” said Wellness Director Lon Keiffer.
Patients receiving home care will be given the technology and training to assess whether or not they need to come in for care, allowing them to avoid the extra trip and have a better grasp of their health from home.
